I am in Europe and annually shell out for a subscription to MLB.TV, but not the "premium" version. In the cheaper version I get whatever coverage is local for the home team. You may see an advantage in this, in that I only have to endure Michael Kay when the Yanks are in the Bronx, but some of the local coverage is beyond abysmal. Last night (the game finally started after 11:00 p.m. here), they spent more time focused on the broadcast booth with McCarver and Bob Gibson than they did on the field. I had to mute the audio feed and switch on The Master, which was a bit odd as WFAN is not in sync with the MLB video. Whatever: the Yanks won!
